DIRECTIONS:  Each Graduate Interdisciplinary Program (GIDP) must conduct periodic reviews of the faculty members in the GIDP.  This Committee on Global Change Faculty Activity Report Form has been designed to facilitate this review.  Please take a few minutes to answer the 9 questions below. To complete your report, click SUBMIT at the bottom of the page.

After we receive your submission, a letter will be sent to your unit head acknowledging your contributions towards the Global Change Ph.D. minor program. Thank you for your ongoing support of the Global Change GIDP!

NOTE:  You are being asked to complete this form because -- at some point in the past -- you were appointed to The Committee on Global Change by the Director of the Graduate Interdisciplinary Programs.

1. After reviewing the Membership Criteria & Responsibilities of Global Change GIDP Faculty, please indicate your interest in continuing in your appointment to The Committee on Global Change by selecting one of the choices below:
